Goal 1: Create a process through which any person contacting a community partner will receive a culturally competent, appropriate response to identify needs.

  • Strategy 1: Ensure that community partners have and use a sound needs assessment and referral process.
  • Strategy 2: Ensure a seamless sharing of information between community partners that maximizes appropriate use of technology.
  • Strategy 3: Ensure that community partner staff members who have first contact with clients receive training in the provision of responsive and culturally competent interactions.

Goal 2: Create a process through which community partners make appropriate referrals to services or providers to address the needs identified.

  • Strategy 1: Engage stakeholders in development of the process.
  • Strategy 2: Implement the process throughout Tarrant County.

Goal 3: Create a process through which any person contacting the appropriate community partner receives services in a competent and timely manner.

  • Strategy 1: Ensure that clients experience a warm, caring and clinically and culturally competent intervention.
  • Strategy 2: Ensure that community partners develop and provide interim services when “wait times” are lengthy
ENGAGEMENT

Goal 1: Support the active participation of individuals and family members throughout the behavioral healthcare system.

  • Strategy 1: Create opportunities for community partners and family members to share resources and experiences.
  • Strategy 2: Create environments for family members to feel safe and opportunities for family members to learn from and support one another.
  • Strategy 3: Create communications that are culturally competent, understandable and free of professional jargon.

Goal 2: Integrate family perspective into all organizational levels of Mental Health Connection, including decision-making and policy development.

  • Strategy 1: Identify and engage family members; assess interests, needs and options for participation.
  • Strategy 2: Provide training for family members in peer mentoring, organizational participation and leadership, legislative advocacy, system transformation, and cultural competence.

Goal 3: Within five years, all providers/agencies will have implemented a highly individualized transition planning process.

  • Strategy 1: Initiate transition planning early in the treatment process and identify individual barriers to continuity of care
  • Strategy 2: Evaluate and assess best practices for aftercare services
  • Strategy 3: Establish a framework of transition planning for all agencies (traditional and non-traditional stakeholders)

  • Strategy 3: Assess current technology, identify and recommend changes that support collection of data for identified outcomes.

Goal 2: Utilize data, evaluation and outcomes to determine the effectiveness of the Tarrant County behavioral health system.

  • Strategy 1: Link community partner transformation efforts to system outcomes.
  • Strategy 2: Identify, review, aggregate and evaluate existing data to inform ongoing and future work.

Goal 3: Communicate system-wide indicators and outcomes to the community at regular intervals.

  • Strategy 1: Develop an outcomes communications plan that involves dissemination and feedback.
  • Strategy 2: Implement, monitor, evaluate and modify the communications plan.

Goal 1: Develop, adopt and operationalize a set of guidelines for quality behavioral health care specific to level of care.

  • Strategy 1: Establish a task force composed of administrators, practitioners, policy makers and family members to identify guidelines.
  • Strategy 2: Communicate the recommended guidelines of quality behavioral health care throughout the community.

Goal 2: Establish and encourage community partner accountability for continuous quality improvement.

  • Strategy 1: Develop and provide training, tools, coaching, and incentives for continuous quality improvement in community partner organizations.

  • Strategy 1: Engage community partner leadership to operationalize shared values and goals through cross-agency training, shared data systems and client-focused teams.

Goal 2: Establish partnerships with traditional and non-traditional community partners to increase awareness of behavioral health issues.

  • Strategy 1: Identify and engage traditional and non-traditional stakeholders not currently involved in Mental Health Connection.

  • Strategy 1: Infuse the vision, mission and goals of Mental Health Connection into the awareness, knowledge and practice of its partners and the community at large.
  • Strategy 2: Conduct a needs assessment to identify needs and priorities for system transformation.

Goal 2: Use the existing structure of Mental Health Connection to organize, support and monitor development and implementation of the strategic plan.

  • Strategy 1: Align the work of the committees and platforms.
  • Strategy 2: Identify needs and secure resources for implementation of the strategic plan.

Goal 3: Organize and articulate an advocacy agenda and identify and pursue funding for community programs and services.

  • Strategy 1: Identify, establish relationships with and provide support to legislators on committees that have a role in behavioral health policy or funding.
  • Strategy 2: Establish a team tasked with identifying and coordinating grant applications and other funding opportunities

Goal 1: Identify, recruit and support a high-quality workforce that views human service work as desirable, motivating and satisfying.

  • Strategy 1: Establish a Career Development Task Force that focuses on work with public and higher education to stimulate interest in behavioral health professions.
  • Strategy 2: Establish a task force of community human resource leaders to address issues of workforce recruitment and development.
  • Strategy 3: Complete a compensation and benefits market study.
  • Strategy 4: Build support for increased compensation and additional benefits for workers.
  • Strategy 5: Develop and pilot creative compensation and incentive plans.

Goal 2: Improve service quality through the provision of training and support to the workforce.

  • Strategy 1: Establish a community-wide approach to professional development and training through shared resources.
  • Strategy 2: Develop and implement a peer mentoring model that community partners can use to support new service providers.
  • Strategy 3: Identify and secure funding for professional development in best practices that is delivered by recognized authorities.

Goal 3: Improve service quality through a well-informed, culturally and linguistically competent and engaged workforce.

  • Strategy 1: Identify, evaluate and access curricula and training in practices that are culturally and linguistically competent, evidence-based, family-focused and strengths-based.
